(We remind you that if the information you provide about the requested document is incomplete, the institute will not be able to satisfy your request: in this case, you can order us a guided search by filling in the proper form so that to integrate your request with the missing information).

I authorise Marsilio Editori to request the document reproduction request form from the Archivio di Stato di Venezia on my behalf. I understand I will be personally responsible for the use I intend to make of the requested form. I also thereby agree that:

I entrust Marsilio Editori to fill in the form with the information on the material I need the reproduction of as previously agreed between Marsilio Editori and me; Marsilio Editori will send me the form to sign;
I will personally fill in the form, sign it and then send it back to Marsilio Editori which will take care of the reproduction operations.
